Please review and bring the appropriate documents with you on your first day of employment.  
In addition to the below forms/documents, be sure to bring any proof of education (such as a diploma or transcripts), any relevant licenses, certifications, driver's license, etc. that are required for the position you've been hired for.
  • Direct Deposit - Be sure to bring a voided check or verification of account to set up your direct deposit.
  • Federal W-4 - Review and be prepared to complete both a State and Federal W-4 on your first day.
  • I-9 Employment Verification - Bring with you unexpired accepted documents from page three of this document.
  • State W-4 - Review and be prepared to complete both a State and Federal W-4 on your first day.

Employees requesting FMLA leave may require a medical verification to support certain FMLA-qualifying reasons. To request FMLA, contact Human Resources at 218-312-1574.

Completed forms must be returned to Human Resources.
