In addition to the below forms/documents, be sure to bring any proof of education (such as a diploma or transcripts), any relevant licenses, certifications, driver's license, etc. that are required for the position you've been hired for.
- Direct Deposit - Be sure to bring a voided check or verification of account to set up your direct deposit.
- Federal W-4 - Review and be prepared to complete both a State and Federal W-4 on your first day.
- I-9 Employment Verification - Bring with you unexpired accepted documents from page three of this document.

The City of Hibbing has adopted a Drug, Alcohol, and Cannabis Testing policy in accordance with MN. Stat. § 181.950 - §181.954 for job applicants and employees. Copies of the policy are available for inspection by job applicants or employees during regular business hours in the city’s Human Resources office.

Employees requesting FMLA leave may require a medical verification to support certain FMLA-qualifying reasons. To request FMLA, contact Human Resources at 218-312-1574.
- Employee’s serious health condition, form WH-380-E – use when a leave request is due to the medical condition of the employee.
- Family member’s serious health condition, form WH-380-F – use when a leave request is due to the medical condition of the employee’s family member.
Completed forms must be returned to Human Resources.
